signature. Also fill in the blank spaces above your signa-
ture, showing the date on which you signed the permit. All
signatures shculd b4 in ink. Return all copies to this office
for further approval.
After the permit has been e.pproved by the officials of this
Department, a fully execrated copy shall be returned to you
for your files.
