City of f~tntean Illinois, that the Central Illinois Public Service
Company be and hereby it is requested to furnish the t~.ty of C~trrr~
Illinois, the following street lighting service, effective as completed:

The additional service hereinabove referred to is requested pursuant to the provisions
of that certain written agreement heretofore entered into between the Company and the
