WHEREAS, a permit from this Department is required before said work can be legally

THEREFORE, be it resolved by the

damage that may occur to persona or property, during the prosecution of such work, and
assume all liability for damage to persona or property, dua':- to accidents or otherwise,
by reason of the work which is to be performed under the provisions of said permit.

permission and authority to do certain work herein described on the right-ofway of the State highway knoam u
Route S.A. 5 ,Section Y-15D ,from Station __ to Station ___,
County. The work is described in detail on the attached sketch and/or as follows:
Permission and authority is requested to locate, construct, operate and maintain
a 14 inch ductile iron sanitary sewer force main encased in 34 feet of 24 inch steel
casing across and under the pavement of Route State Aid 5 (Chestnut St.) Section Y-15D
at Bradley Corners and constructed in accordance with the attached print and State
Specifications Governing Boring or Jacking.
The backf ill of the trench shall be thoroughly compacted at time
of backf ill by water soaking or tamping by mechanical means, and
any depressions which may develop within the area involved in the
construction operations, due to settlement of the backfilling mat-
erial, shall be kept filled, and the highway restored to its orig-
inal condition, duringg a period of 6 months from the date of the
completion of the work authorized in this penait.
It is understood that the work authorized by this permit shall be completed within 6 mo • after the date this
permit is approved, otherwise the permit becomes null and void. This permit is subject to the conditions and
restrictions printed on the reverse side of this sheet.
This permit is hereby accepted and its provisions agreed to this ~ ~yC~ day of

The Contractor shall furnish the State of Illinois, Department of
Transportation, a surety bond in the amount of $1,000.00, to insure
fulfillment of the provisions as set forth in this permit, said
bond to remain in full force and effect for a period of 5 years
from the date of approval of this permit.
An authorized official of the city shall approve all copies of
the attached Resolution, to do the above described work.
The Petitioner agrees to notify Mr. J. E. Harland, District Engineer,
Department of Transportation, 6035 N. Mount Hawley Road, Peoria,
Illinois as soon as the above described work is completed, in order
that an inspection may be made.
NO WORK IS TO BE STARTED ON THE ABOVE PROJECT UNTIL THE PERMIT HAS
BEEN APPROVED BY THIS DEPARTMENT.
THE PETITIONER IS TO NOTIFY THE DISTRICT TRAFFIC ENGINEER (TELEPHONE;
691-2110) FIVE (5) DAYS BEFORE THE START OF CONSTRUCTION OPERATIONS.
TRAFFIC CONTROL STANDARDS NUMBERED 2300, 2298-3, 2299-3, and 2305-2
ARE ALL MADE A PART OF THIS~PERMIT.
THIS PERMIT SHOULD BE AVAILABLE AT ALL TIMES AT THE PROPOSED PROJECT
IF REQUESTED BY A DULY AUTHORIZED REPRESENTATIVE OF THE DEPARTMENT
OF TRANSPORTATION OR THE STATE POLICE.
