RESOLUTION NO. 3606
A RESOLUTION APPROVING THE GROUP HEALTH BENEFIT PLAN FIRST
AMENDMENT FOR THE CITY OF CANTON
This First Amendment to the City of Canton Group Health Benefits Plan ( "Plan") is made
in duplicate in Canton, Illinois on the date noted below by the City of Canton
( "Employer ")
WHEREAS, the Plan grants the Employer the right to amend provisions of the
Plan; and,
WHEREAS, the Employer desires to make such amendments; and,
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF CANTON, Fulton County, Illinois is hereby replaced as follows:
The City of Canton Medical, Dental and Disability Plan is amended effective May
1, 2002, to add an optional alternate medical sub -plan called the Maxi Plan. The Maxi
Plan is offered to all active employees. The Maxi Plan covers the same benefits plus
100% of drug card deductible. The Maxi Plan does not have a deductible or co- insurance
requirement. Covered expenses are payable at 100% with no deductible or co -pay with a
maximum of $1000 per claim. An individual covered under the Maxi Plan may withdraw
from Maxi Plan at any time and add with written notification to the Contract
Administrator. Dental and Disability benefits remain the same.
The deductible Medical Benefits Section, specifically the subsection entitle Other
Covered Services, is hereby amended to include the following effective May 1, 2002.
(k) Treatment of temporomandibular joint syndrome including treatment with
intraoral prosthetic devices, but excluding treatment for any other procedure to alter
vertical dimension. Benefit is subject to deductible and considered at a 90% benefit level
of reasonable and customary fees and limited to a maximum lifetime benefit amount of
$2500.
The Mayor and City Clerk are hereby authorized and directed to execute and deliver said
agreement on behalf of the City of Canton, Illinois, Fulton County.
That this resolution shall be in full force and effect immediately upon its passage by the
City Council of the City of Canton, Fulton County, Illinois

thereof this 17th day of April '2002.
